OPT Application Steps

OISS will contact you to let you know that your OPT I-20 is ready and to schedule an appointment for you with an advisor at OISS. You must bring in all of your OPT application materials listed below to this appointment for review by the advisor.


Materials to be presented at your appointment with an OISS advisor, after your I-20 has been issued:

  • Original application Form I-765 ( for guidance in completing the form see also our Sample I-765). Employer name and E-Verity number must be included on 17 month OPT applications
  • 2 color passport-style photos. (see photo composition guidlines)
  • $340 fee personal check/money order payable to USCIS. Personal checks are highly recommended!! (see sample check)
  • Copy of valid passport biographical page (with extension page, if applicable)
  • Copy of most recent F-1 visa stamp/status approval notice
  • Copy of I-94 (front and back)
  • Copy of all previous CPT I-20s at current level (if applicable)
  • Copy of previous Employment Authorization Doc (if applicable)
  • Copy of your job offer letter, if you have one (this is not required).*
  • Copy of your degree (only required for OPT 17 month extension applications)

*Under certain circumstances, if you already have an official job offer, OISS can request that USCIS expedite the processing of your OPT application.

During your appointment the OISS advisor will review these OPT application materials and confirm that the application is complete. You will then proceed to
